NOTICE IS HEREBY GIVEN that, pursuant to the agreement by which you hold possession of the premises known as

WITHIN THREE (3) DAYS after service of this notice on you, you are required to pay said rent in full, or to deliver up possession of said premises to the undersigned or his authorized agent, or legal proceedings will be commenced against you to recover possession of said premises, to declare said agreement forfeited and to recover TREBLE RENTS AND DAMAGES for the unlawful detention of said premises.

YOU ARE FURTHER NOTIFIED that the lessor does hereby elect to declare a forfeiture of the agreement under which you hold possession of the premises described above.
